Invitation for consumers and carers to participate in Wellness and Reablement National Review

This survey will close on Monday 26 June 2017

The Nous Group, on behalf of the Department of Health, has extended the date for clients and carers using Government home care programs to have their say in an important national review. The review focuses on wellness and reablement approaches in home care and explores people’s experiences using home care services. Wellness and reablement is about helping older people to live independently, achieve their goals, stay connected to their communities and improve their quality of life.

If you, or someone you know, currently uses one or more of the Government-funded home care programs below, you are warmly invited to complete an online survey.

The survey relates to:

  • Commonwealth Home Support Programme (CHSP)
  • Home Care Packages (HCP)
  • Transition Care Program (TCP)
  • National Aboriginal and Torres Strait Islander Flexible Aged Care Program (NATSIFACP)
  • Short Term Restorative Care (STRC)

The survey is anonymous, and participants cannot be identified.
